Proposal” and
submit to the Contracts Section by fax or mail to the

order a bid proposal you must complete Form "R-125 "Request
for Bidding Proposal" and submit to the Contracts Section by fax or mail to the
address posted below.

site.
- If
you are not ordering a hard copy of the Proposals (Option
1 or 2) and/or Plans, you will not be included on the
Planholder List unless you click on the link below
and fill out the information as indicated. Note: this is not an order form; it only
registers you on the Planholder List.
